Forms for Stewards

Union Fact Sheet
This form is used to document the nature of a grievance through the use of the 5 W’s. It’s available to anyone and should be filled out any time someone wants to grieve an issue.
Grievance Form
This form is used for creating an electronic grievance. Please note this form requires a password to download. You can get this password from a member of the stewarding committee. Stewards, please remember to get a grievance number and document this grievance on the Grievance Register.
Overtime Release (Extended Hours Waiver)
A form members must fill out when a department is voting on an Extended Hours Package.
Extended Time Package
The main extended hours package. You will also need separate part time or full time forms. The main extended hours package alone is not designed for addressing departments with multiple varying lines.
